Data Package

We provide separate Data Packages for producing and injection wells. There are two options for completing these forms:

• Email form — Complete the form online and automatically submit your data to Gel-Tec via email. This option is recommended for projects involving only a few wells.

• PDF form — This option is recommended for larger projects, or for any project where you wish to keep a copy of your completed Data Package. You may download the form in Adobe PDF format for completion and mailing or faxing. Or, if you have a newer version of Adobe Acrobat or Acrobat Reader*, you may also be able to complete the form online and then download for emailing, or print for mailing or faxing. (Note: Each PDF file is about 75kb in size.)
